Waiver & Release of Liability

I, the parent or legal guardian of the player, hereby grant permission for my child to participate in all activities associated with the North Shore Soccer Academy (NSSA).

Acknowledgment of Risks

I acknowledge and understand that participation in soccer involves inherent risks, including but not limited to:

Injuries from falls
Contact with other players
Equipment-related incidents

Release of Liability

I agree to release, indemnify, and hold harmless NSSA, its coaches, staff, volunteers, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any injury, illness, or other harm incurred by my child while participating in NSSA activities.

Health Certification

I certify that my child is in good health and able to participate in all soccer-related activities.

Medical Emergency Authorization

In case of a medical emergency, I authorize NSSA staff to seek medical treatment for my child as necessary.

Tuition Policy

I understand and agree that the tuition fee is non-refundable.

Media Release

I grant permission for NSSA to use photos and videos of my child for promotional purposes, including but not limited to:

Social media platforms
Newsletters
NSSA website
